    Revolutionizing Shoulder Replacement

Dr. Aditya Sai’s Approach Using Patient-Specific Instrumentation

    MS, DNB (Orthopaedics)

At the shoulder & Knee clinic in Powai, a revolutionary approach to shoulder replacement is making waves, thanks to the pioneering efforts of Dr. Aditya Sai. Leveraging the latest advancements in medical technology, Dr. Sai’s approach focuses on the use of patient-specific instrumentation (PSI), a technique that promises to enhance the precision and effectiveness of shoulder replacement surgeries.

    What is Patient-Specific Instrumentation?

Patient-specific instrumentation is a cutting-edge methodology in orthopedic surgery that involves creating surgical tools and components tailored to the individual anatomical details of a patient’s joint. This is typically facilitated by pre-operative imaging techniques such as MRI or CT scans, which provide a detailed blueprint of the patient’s anatomy. These images are then used to design and manufacture instruments and implants that are uniquely suited to each patient.

The Benefits of PSI in Shoulder Replacement

The use of PSI in shoulder replacements offers numerous benefits over traditional methods:

    1. Enhanced Precision: Custom tools guide the surgeon to cut and align the implants more accurately, which can be critical in the complex structures of the shoulder.

    2. Reduced Surgery Time: With instruments tailored for specific surgeries, the procedure can often be performed more quickly, reducing time under anesthesia and potentially lowering the risk of complications.

    3. Improved Outcomes: More accurate implant placement can lead to better joint stability and range of motion, which significantly improves the functional outcomes for patients.

    4. Faster Recovery: Precise alignments and cuts typically result in less tissue damage, which can lead to a faster and less painful recovery.

    5. Lower Risk of Revision Surgery: Improved accuracy in the placement of implants may also reduce the need for future corrective surgeries.

Dr. Aditya Sai’s Role in Patient-Specific Instrumentation

Dr. Aditya Sai who handles the Sports injuries ,Shoulder surgery & Arthroscopy portfolio has been at the forefront of adopting PSI for shoulder replacements. His dedication to integrating state-of-the-art technology with clinical practice has set a new standard in optimising patient outcomes. Dr. Sai’s expertise not only lies in performing complex shoulder  surgeries but also in customizing the treatment to fit the unique needs of each patient, thereby ensuring optimal outcomes.

    Case Scenario for PSI Technique

Mr Panchamia had been suffering from shoulder pain post a shoulder surgery done for fracture  in Gujarat  since 2021 . He subsequently developed infection of the shoulder . After detailed discussion with Dr Sai further treatment course was charted . In the first stage of the surgery the previous infected implants were removed , joint was cleaned and a antibiotic eluting  device was implanted. Once the infection got cleared in the second stage a revision implantation was planned . The problem faced was the previous surgery and subsequent  infection had destroyed significant bone . Hence the need arose for doing something different. For planning the surgery dedicated CT scans were done and the implant dimensions / features were calculated . A customised jig/ bone targeting device was designed specific to the patient’s anatomy and bone loss. This is done generally in leading centres in Europe and the USA but a technology being offered at less than a handful centres in India  . Post the customisation the second surgery was done and the implantation was carried out . Mr X had a smooth uneventful recovery post the procedure.

    Looking Forward

The success of patient-specific instrumentation in shoulder replacements at Dr. L H Hiranandani Hospital points to a broader trend towards personalized medicine in orthopaedics. As technology advances we are one of the very few centres in India which provide this standard of care.
